All patients must complete the digital check-in prior to arrival.
At each appointment, please bring photo ID and Insurance card.
Patients who are unable to provide this information may be
subject to a rescheduled appointment.
nEW PATIENTS
New patients are encouraged to complete the initial new patient registration forms prior to their first visit. Completed forms may be faxed or brought into our office. Our fax number is 678.750.4005. If faxed, please bring the original document to your first visit along with your photo identification and insurance card.
EARLY ARRIVAL
Patients who are unable to provide this information prior to their first visit are asked to arrive 10-15 minutes early. A copy of your child’s medical records should be faxed to the office prior to your first appointment.
