Most frequently asked questions about Monkeypox
Masks are required during all in-office visits. Our goal is to ensure that you have a safe, exposure free encounter when visiting the Pediatric Center for Wellness.

Call our office at the end of the quarantine to speak with RN/Provider to confirm your child has no symptoms. At that point, excuse can be given. Regardless of test result, patient must be fever free for 24 hours prior to returning to school. If asymptomatic AND negative test 5 days post exposure, may return to school.

Obtaining a referral requires an initial consultation with the provider. Following the consultation, the provider will inform one of our referral specialists to fax the referral to the preferred officeWe will contact the parent/guardian with the contact information of the preferred office within 24-48 hours. 

To request a prescription refill, please contact our Nurse by calling our phone number (678) 750-4000 and choosing the option to speak with the nurse. If you are not able to reach the nurse, please leave a detailed message with your child’s first and last name, Date of Birth, the name of the prescription and the best contact number. The turnaround time for a call back is typically 24-48 hours. 

If the parent/guardian would like to request medical records for person use, they must call our office to request their child’s medical records which will incur a $30 fee. These records may be picked up in person or sent via mail. If the parent/guardian would like to request medical records to be sent to another practice, they must complete a medical record request form. The turnaround time will be 8-10 business days. 

The parent/guardian must fill out a medical record release form and return it to us so we are able to fax it to the requested office. The parent/guardian can also go to their previous office to request the medical records. 

The parent/guardian must call to have the form completed if the patient has had a Well Child Visit within 6 months, which will incur a $15 fee. The turnaround to retrieve the physical form will be 5-7 business days. If the patient’s Well Child Visit has been longer than 6 months, you must schedule an in-office appointment for the sports physical, or it can be completed at the next Well Child Visit. 

If the legal parent or guardian chooses to allow someone outside of themselves to bring their child to an appointment, the parent/guardian must fill out a consent form including that person’s information. Once that person arrives, they must present a physical form of ID.
