At Pediatric Center for Wellness (PCW), we strive to make each visit a pleasant experience for our patients. Our new patients are encouraged to complete the initial New Patient Registration Forms prior to their first visit in an effort to shorten the wait time during the visit. Completed forms can be brought in or faxed to our office at 678.750.4005 (If faxed, please bring the original to your first visit) along with a copy of the front and back of the child’s insurance card. Patients who are unable to provide this information prior to their first visit are asked to arrive 20-30 minutes early.

We accept most major insurance plans. If your plan requires you to identify a Primary Care Provider (PCP), then you will need to call your insurance company and switch to Pediatric Center for Wellness as your PCP prior to your visit. Please be aware that this can take up to 30 days.

In most cases, newborn infants are usually covered under the mother’s insurance plan for the first 30 days. New mothers will need to add their baby to their plans before the baby is 30 days old.

Appointment Scheduling

At Pediatric Center for Wellness, our goal is to see patients on time and keep your wait time to a minimum. To achieve this, we require that all appointments be scheduled prior to visiting our office. To help us schedule your appointment appropriately, please let us know the nature of your concerns and if you believe you will require additional time to discuss further issues.

If you require that an additional child be added to the schedule, please call the office to let us know. Appointment times may need to be changed in order to accommodate the additional child.

While we want you to feel comfortable with both providers in the practice, we recommend choosing one of us as your primary pediatrician. We feel that continuity is important, and that having the same provider for most of your visits helps to improve the quality of pediatric care we can provide for your child.

Appointments will be verified on the business day prior to your scheduled appointment time. During this confirmation call, you will have the ability to cancel or reschedule your appointment. Appointments can also be canceled or rescheduled 678-750-4000.

Late and Missed Appointments

We know that your time is important. To ensure that you have sufficient time with your doctor, we do not routinely overbook appointments. To keep our schedule running as smoothly as possible, we request that you arrive on time. If you are unable to make your appointment at the scheduled time, please call our office to let us know.

If you are going to be late, please let us know as soon as you can. If you are going to be more than 15 minutes late, we may need to reschedule your appointment in order to continue to see our other patients in a timely fashion.

While we will not charge for a first missed appointment, if there are multiple missed or late appointments, we reserve the right to charge a $25 fee to your account.

After-hours Calls/Emergencies

If you believe you have a life-threatening emergency dial 911 or go to the nearest emergency room.

If you feel that your child needs to be seen in the emergency room, we recommend using the facilities at Children’s Healthcare of Atlanta (either Egleston or Scottish Rite). We feel that the services provided at these hospitals are better suited for children. Please call our office line, 678-750-4000, to let us know that you are going to the emergency room so that we can follow up with you and/or the emergency room physicians as needed.

We understand that problems or questions about your child do not always occur during normal business hours. Therefore, there will always be a pediatrician on call in case of an after-hours emergency. Your call will first be answered by our answering service, which will direct you to your insurance nurse line if appropriate. If, after you speak with the nurse, it is determined that you need to talk to the on-call pediatrician, the answering service will page the doctor. As these services are not perfect, we ask that you call the answering service again if you have not received a call back within 30 minutes.

  • If there is no response after 30 minutes, please call back.

Routine Calls and Questions

We request that routine questions wait until the office is open, and that our after-hours line be used for emergencies only. We are also accessible for questions during office hours. We will be checking these messages routinely during regular office hours. However, we will not be checking messages routinely after-hours, so this method of communication should only be used for non-urgent questions.

New Patient Checklist

  • If your plan requires a PCP, call your insurance company and add Pediatric Center for Wellness as the PCP.
  • Complete the New Patient Registration Forms
  • Fax complete registration forms and child’s insurance card to our office at 678.750.4005. Please bring in originals to your first visit.
  • A copy of your child’s medical records and immunization records.
